Our intake &quot;form&quot; is a strip of paper with size and serial number on it, stapled around the handlebar of bikes deemed worthy of repair.  This is the last step of <a href="http://ohiocitycycles.org/wiki/tiki-index.php?page=Prep+bikes+fro+repair+or+salvage">prepping a bike</a> for repair, and is replaced with a string price tag when the <a href="http://ohiocitycycles.org/wiki/tiki-index.php?page=TuningForSale">bike is repaired</a> and entered into inventory.<br>

Ok, so somehow Free Ride has gotten by 8 years and thousands of EABs without a good bike intake form.  <br><br>Crazy huh.  Does anyone have any really detail, well organized ones that can be easily attached and displayed on the bike?  <br>
