At BikeFarm we have just finally now submitted our forms, and like many others...set it up how "they" want it... and disregarded that format in practice, but good luck.... Also, we had co-presidents.... They wont allow for that.... so we made someone CEO and someone Vice....Its more likely they'll approe something familiar, adn thats why we went with the "standard" format.
