I was really hoping to see someone else's response to this, but I'll contribute the bit of experience we have had here.
In all of our programs, we have tried to set up a referral agency or used existing criteria to identify need rather than coming up with our own application.
We build bikes and bicyclist, and we do a decent job of that. We are not experienced or very good at determining level of need or qualification for financial assistance. So we ask the WIC program, the shelter, Goodwill, and other orgs that already do this to refer people to our bike classes. It works great for us.
You might be able to use free-and-reduced lunch as a qualifier for scholarship, especially if the school can help verify that. Or have the school provide you with referrals for scholarship.

Bici Centro is going to offer a Youth-Earn-A-Bike program in several junior high schools in Santa Barbara. We want to offer scholarships for low-income families who can't pay. Does anybody have some examples of application forms to share?
