Attached is a waiver from the Salt Lake City Bicycle Collective. It almost
certainly won't match your needs or situation perfectly, so after you
modify it to reflect your operation, you should consider having it reviewed
by an attorney.

easy to read/understand. I've tried searching through the archives of the
