I don't have one handy, But in Davis they call it a Safer Space Agreement. Safer rather than safe because it's a goal, something to be always worked on. Agreement rather than policy because by entering the shop you agree to contribute to a safer space.

We’d like to implement a safe space policy in our shop similar to the one that was developed at Bike!Bike!. Would somebody mind resending the Bike!Bike! safe space policy? Can anyone share a safe space policy that they have in their shop or some general advice on things that work well (or poorly)?
