I'd love to see the PDF as well. We're looking at developing resources to address all elements of group rides including:
- Large gaps in experience between riders (buddy assigning)
- Agreements on signaling 
- Building confidence with new riders
- Group management (ie, when you have very different skill levels)
- Mechanical issues 
- Other positions besides ride leader (sweeps, floats, center breaks, corner markers, traffic blocks)
- Managing rides with few experienced riders 
- Illegal courtesies 
- Traffic management 
- First aid
- Emergency management 
- “Failure” to complete ride
- Feedback
Copenhagen left turn
- Bad behavior of riders
- Route design 
- Pace setting

London Cycle Link has a group working on protocols for marshaling and group rides. So far it's a bit heavy on legal/rules of the road stuff, and a bit light on best practices and tips for organizing rides and marshaling. Does anyone have a document or resource they could share?
