Congrats on all the bikes!!

Omar at PEDAL recently sent us 2 examples of their consent form but I wouldn't want to distribute or post to the wiki without permission.  Perhaps they would allow me to upload them making sure credit is given as due?

It makes sense to start a waiver/permission slip category on the Wiki, though I'm not sure of the most appropriate place for it (insurance?? legal??).  Anyone with the documents to share could upload them to that page for everyone to see.
