Last year's bikebike site had a handy listing of bike co-ops around the country that had attended the conference, and if they had a website, the listing was an active link. Is there such a directory this year? At the bicycle collective network site, it seems not all the co-ops are represented, not even close. Can bikebike make another such directory of websites and co-ops. Alternatively, how does one get listed in the BCN website?
It is up to each shop/collective to add themselves to the BCN. If you follow the link to the directory in the main menu or from along the top, you should see a link in the main content section for "Add Entry". If you want it to show up on the map, you have to add the latitude and longitude.
As far as your other questions, not sure.

You can add un-modifyable entries anonymously. However it is better if you create a login and then add the entry -- that gives you the ability to change it later.
